To register for an account you must be enrolled in an assistance program through PAN. You will need your Member ID or SSN and the patient's last name and Date of Birth to begin registration.

To complete the registration, please confirm or enter a valid email address. This email will also be the patient's username. A registration email will be sent to this address with the patient's temporary password.
